The Secure Banking Certification Form is a form that is required by VIP Lounge for all players making deposits by Credit Card to have on file. This form is used strictly as a tool to prevent fraudulent activity and will be used, when necessary, to protect each player as well as the VIP Lounge Casino. Having this certification on file with the VIP Loung will ensure that all of your credit card deposits are fast, easy and hassle free. Not only will having your account certified make depositing a breeze, it will make cashing out just as easy. It will also make you eligible for higher banking limits, tournaments and contests and also give you preference in regards to making withdrawals with the VIP Lounge Casino.

Once you fill in the automated options provided in the form you must print it and fill out the remainder by hand. It is necessary, for your security as well as ours, that you sign this document by hand.

If you are not able to scan and email this document back to us (recommended) and must fax it back to us (not recommended), please keep in mind that you are responsible for making sure that this fax is legible. If, after review, it is not found to be clear or legible, this will be denied and you will have to resend it.

Make sure to sign (NOT initial) each page if you are sending these documents by fax.

All forms must be accompanied by a copy of a Government Issued ID (state issued drivers license or passport is okay) for age verification. If you have ever deposited using a Credit Card in the past you will be required to send a copy of all Credit Card(s) that you have used with the VIP Lounge (front and back).

Please complete one form for each credit/debit card you have used with VIP Lounge (i.e. multiple forms may be required).
